Introduction

This page assumes that you already know how to download files from the Internet and view or print them. If you are unfamiliar with downloading, viewing, or printing these files, please peruse the Forms Help Info page.

The documents listed in the sections below are available for download. You may download any form by clicking on the link corresponding to the desired file format listed below. You may either open a form or save it to disk; however, we strongly recommend that you save to disk. All documents linked on this page are under 125 kB in size, except as otherwise noted, and most are significantly smaller.

LATEST Course Outline Forms

This form has now been officially "blessed" by the curriculum committee and should be used for all courses, effective immediately. This version incorporates all suggestions for change that were brought forward at the 29 August 2001 meeting of the curriculum committee. It also includes a number of subtle modifications approved since that time, and is subject to continuous revision as new issues arise. It is designed to work with the Course Outline Cover Sheet Form described below; these should be completed and submitted together when adding or modifying a course.

Please note that course deletions may now be accomplished using only the Cover Sheet Form; it is no longer necessary to submit a copy of a course outline form to delete a course.

If your course has a distance education element, please also complete the Distance Education Course Appendix (links given in Supplemental Forms below). If your course has a prerequisite, corequisite, or advisory, please also complete the Prerequisite/Corequisite Justification Appendix (links given in Supplemental Forms below).

This is a document that contains summary information, articulation data (for transferable classes), alphanumeric codes for the state chancellor's office MIS system, and the official approval signatures for a course outline. A completed copy of this form must accompany each course outline presented to the curriculum committee for approval. This form may also be used by itself to effect the deletion of a course; it is not necessary to submit a copy of the course outline form to delete a course.

Supplemental Forms

Bullet  Distance Education Course Appendix:     DOC   |   PDF

This is an addendum, or appendix, that must be submitted with any course outline produced for a course that may be taught using a distance education modality (e.g., a television or Internet course). The curriculum committee has approved this addendum for temporary use until it establishes a formal procedure for approving distance education courses. The addendum provides a checklist of items that the outline author should verify. Course outline authors who present new or updated distance education courses to the curriculum committee should be prepared to discuss how the checklist items have been or will be satisfied.

This is an addendum, or appendix, that must be submitted with any course outline produced for a course that specifies a prerequisite, corequisite, advisory, or limitation on enrollment.
